This assessment is intended to examine an individual’s use of alcohol and/or drugs and determine the impact that substance use may have on their health, relationships, daily functioning, and legal or occupational responsibilities. The assessment helps identify the presence and severity of a substance use disorder and provides recommendations for appropriate treatment or intervention.
Areas commonly assessed include:
Current and past use of alcohol, prescription medications, and illicit substances
Frequency, duration, and patterns of substance use
Signs and symptoms of substance use disorders
Impact of substance use on work, school, family, and social functioning
Mental health concerns that may contribute to or result from substance use
History of treatment, recovery efforts, and relapse
Motivation and readiness for change
Risk factors and protective factors related to recovery
Legal, occupational, or child welfare concerns related to substance use
Physical health concerns associated with substance use
The assessment is a clinical interview with the client. Collateral information will be reviewed, as appropriate. Findings are summarized in a written report that may include diagnostic impressions, level-of-care recommendations, treatment recommendations, and other relevant clinical opinions. This is a therapeutic service that will typically be covered by insurance.
Please note: Individuals can be referred for substance use assessments by self, healthcare providers, attorneys, courts, probation or parole agencies, employers, educational institutions, or child welfare agencies. This assessment is NOT a forensic service. We will refer examinee’s to appropriate services at the conclusion of the evaluation, if requested.
